Before You Begin…

Here’s some important information to know before you fill out the form below:

Dear Parents,

Jesus Christ entrusted the Sacraments and the Gospel to his Church and the laws of the Roman Catholic Church. Therefore, to ask someone to be a Godparent for your child is a sacred honor. In fact, spiritually, in infant Baptism, the Godparent, together with you the parents, will present the child to God promising to help this little infant to grow and to live a Christian life by fulfilling faithfully the obligations inherent to it. Only one male or one female Godparent, or one of each gender, is to be employed. The qualifications of Godparent for the Catholic Baptism of an infant must be as follows: The Corpus Juris Canonici canonical law since the middle of the 12th century (1300 AD.) and The Code of Canon Law, promulgated in 1917 AD and 1983 AD (Canons 872 & 874) list the requirements that a person must possess in order to qualify as a Godparent in Baptism. It states that one must not be bound by any canonical penalty legitimately imposed or declared: Accordingly,

  1. A Godparent:

    1. Must be a Roman Catholic and NOT younger than 16 years old.

    2. Must have already received the Sacrament of Confirmation.

    3. Must not be the father, mother or legal guardian of the child and/or candidate.

    4. Must be a Roman Catholic leading "a life in harmony with the faith and the role to be undertaken." (This means that the person is in good standing and living and fulfilling faithfully, the obligations connected to the Catholic Church)

    5. If the Godparent is married, to qualify as a Godparent, a Catholic priest must have officiated at the wedding.

    6. A Catholic in a civil marriage, or married by another minister, or married by the Justice of the Peace cannot serve as Godparent for this does not fulfill a life in harmony with the faith.

    7. Must not be cohabiting or living together.

  2. A Witness:

    1. A person who belongs to a non-Catholic ecclesial conformity may not be a Godparent at the Baptism of a Catholic, but may be recorded as a witness together with a Catholic Godparent. (A Witness is one who has never been baptized Catholic).
